How are factory molds made?

Factories that make plastic things, such as toys or parts, use molds to make it perfect each time. But how exactly are these molds made in the first place? How do they get them so perfect?

My guess is that they first model the space in a 3D program then use lasers to hollow it out. Please give as much information as you can.

Tool and die shops make injection molded plastic molds typically using 3D programs to get tight tolerances between cavities. The type of injection molded plastic dictates the closeness of the machining of the molds. Machining is typical other than the high precision areas that may use diamond tooling to get better surface finish. Coatings are also then applied to extend tool life and/or protect the mold from chemical attack from more aggressive materials. Look at the plastic part, you may be able to see a split line (where the two halves of the tool close) and also look for any knockout pin positiions. They are usually small raised or depressed circular sections where knockout pins come through the mold to release the part from the mold. The better the tooling, the smaller the witness lines and protrusion of the knockout pin marks. But also, that makes for a more expensive mold and end part too. Good designs also lead to good parts that may not readily show any witness marks from the tooling, and sometimes leave you scratching your head. Bad designs, bad tooling, or bad molding are very evident.
